Provide the 4 pieces of information shown below:
*Note: Claims are submitted under a supervisor’s name as a standard practice. For example, for Alicia and Brittany you will provide the name of their supervisor, Karen Pendleton.

55 mins | $145 - Individual therapy focused on healing and personal growth.
Talk Therapy
60-90 mins | $250 - New Client: Comprehensive Holistic Evaluation
60 mins | $200 - Approximate rate depending on services provided for ongoing follow-ups, care, etc.
30 mins | $125 - Approximate rate for medicine management follow-up.
